Declarations & Consents

Drop Off & Collection(Required)

I give my child permission to come to, and go when necessary from, the Cathedral without supervision at the beginning and end of supervised choir times.

I will drop-off my child at a suitable time before the required start time and be ready to collect my child at the conclusion time from the Cathedral.

Photography(Required)

I consent to Saint Patrick’s Cathedral taking images and recordings of my child, which may be used wholly or in part in any publication (commercial or otherwise), portfolio or public display, in all forms of media.

I do not give my consent for Saint Patrick’s Cathedral to photograph or record my child.

Mobile Phone (for Schola only)

I give my consent for my child to be contacted on their mobile telephone from Cathedral staff about matters pertaining solely to the Cathedral Choir.

Participation(Required)
